Mounting Holes: Satellite Vias Standard?

Hello,

Are there any standards / guidelines on Satellite Vias of Mounting holes? 

The hole size is given by the screw diameter and the pad size is given by the screw's head / washer but what about satellite vias?

- At which distance from the center should the vias be placed? Under the head? Outside of the head?
- What should the diameter of the vias be? Is it proportional to the hole's dimension?

Satellite vias are usually used for mounting holes that need extra support for mounting hardware. 

When using torque to screw to a nut with washer you might need the extra support to prevent crushing the FR4 glass. 

Without this support of satellite vias, you might crush the FR4 glass with too much torque. 

Also, because screws going to mounting holes might need to be grounded, the satellite vias provide extra grounding support.
