I have had to look at some TO-220 (Vertical) recently, and had a look at what PCB Footprint Expert recommended for this device, it seems that PCB Footprint Expert recommends the pins formed from the body different that we get from the factory.
 
eg: PCB Footprint Expert recommends:-
 
 
1 3 5
 2 4
 
 
But we get from the factory already formed as:-
 
 2 4
1 3 5

 
I have had a look at as many producers as I could find for this kind of device but could not find one that was showing like PCB Footprint Expert recommends.
 
Any comments?

Just swap the values for 'D' and 'D1'.
You can do the same thing in the horizontal TO-220 by making 'd' a negative value
