What category would I use to create this part?

SON with Thermal. 

The package has 2 Thermal Tabs but you only create one. 

Then move the Footprint to FP Designer to relocate (offset) the one Thermal Pad (pin 9) and add the second thermal pad (pin 10). 

Then update the Specifications panel with the mfr. name, Case Code, Part Number and select OK.

The select Add to Library.
